Job Description

๑. ให้ความช่วยเหลือผู้ป่วยที่มีปัญหาเศรษฐกิจ สังคม อารมณ์

๒. การส่งเสริมพลังทางสังคม ดำเนินการเพื่อป้องกัน แก้ไข ฟื้นฟูและพัฒนาศักยภาพ ผู้ป่วยและครอบครัวที่ประสบปัญหาให้สามารถช่วยเหลือตนเอง

๓. พิทักษ์สิทธิ ให้ความรู้ การปรึกษาและประสานบริการตามลักษณะปัญหา

๔.เข้าร่วมทีมสหวิชาชีพในการดูแลผู้ป่วยแบบองคืรวม

๕.ส่งเสริมให้ผู้ป่วยดำรงชีวิตอย่างมีคุณภาพ โดยประสานชุมชน เครือข่ายทั้งภาครัญฐและเอกชน เพื่อการติดตามและดูแลผู้ป่วยต่อเนื่อง
